I think any project's delivery relies mostly on the manager, teams need a strong leader who will take control and knows when something isn't working and a new angle must be tried
The use of a team scoreboard helps a lot on this issue. A nice tool for the project leader to grasp misalignment in team expectations and to measure objectively the team feeling in any area that your scoreboard exposes.
The perfect thing about this is that it can all be done with a simple and free online form like google offers
